Clinique Chiropratique Notre-Dame is committed to protecting your privacy and the personal information it collects through its website or by any other technological means, and that it uses or discloses to third parties in the course of its activities.

This Privacy Policy sets out how Clinique Chiropratique Notre-Dame protects the personal information it holds, in order to meet the requirements of the applicable personal information protection legislation to which it is subject, as well as any other applicable laws and regulations in this field.

Clinique Chiropratique Notre-Dame ensures that its employees, and also the third parties with whom it does business, protect the confidentiality of the personal information to which they have access, whatever the medium on which it is held or the form in which it is accessible.

First name, last name, email address, telephone number, IP address, browser type.

Two forms on this website send information to the clinic: the contact form and the appointment request form. They collect your name, email address, telephone number, the message you write, the language of the page, and — on an appointment request — whether you indicated this is your first visit.

Completed new-patient forms. If you are a first-time patient you may attach your completed admission forms to an appointment request. These documents contain health information and are handled as follows:

• They are uploaded to this website over an encrypted connection (HTTPS), then sent on to the clinic by email. The clinic's mailbox is hosted by Google Workspace, so the message travels over the internet to Google's servers. Mail between servers is normally encrypted in transit, but email is not end-to-end encrypted. If you would rather not send health information by email, bring the forms to your appointment on paper instead.
• They are never stored on the website. Each file is checked, attached to the notification email, and the temporary copy is discarded when the request finishes. No copy remains on the web server.
• Once delivered, the documents live only in the clinic's mailbox and in your own patient record, and are kept for the period described under Durée de conservation.
• Attaching the forms is optional. You may bring them to your appointment on paper instead.

A record of each form submission — your name, email, telephone and message — is also kept on the server so the clinic can trace a request that did not arrive by email. These records are deleted automatically after twelve months, and no IP address is recorded. Attached documents are never part of this record.

You may ask us to delete a form submission or an attached document at any time, by writing to the person responsible for privacy named below.

We use the personal information collected solely to achieve the purposes described below.

Clinique Chiropratique Notre-Dame obtains your consent before collecting, using or disclosing your personal information to third parties, except in the cases provided for by law.

Under no circumstances will Clinique Chiropratique Notre-Dame use or disclose personal information without your consent unless it is authorised or required to do so under the applicable legislation.

We retain your personal information for a period of seven years following its last use, except where the law provides for a different retention period.

We protect your personal information by security measures — physical, technical or administrative — appropriate to its nature, so as to prevent anyone from accessing it, using it or disclosing it to third parties without authorisation, and to prevent its loss or any other breach of its protection.
